Job Summary

The Case Manager/Health Educator addresses the health and social service needs of patients and their families, with a focus on migrant/seasonal farmworkers, by helping them to navigate and access community services and resources.   The Case Manager/Health Educator works in the clinical setting, in the community and in farmworker camps.

Responsibilities and Duties

• Educates patients to assist with the adoption of health behaviors.
• Collaborates with clinical staff to facilitate an integrated approach to care coordination and community outreach.
• Provides social support and advocates for patients' health needs.
• Assists patients with accessing basic health services such as transportation and care coordination.

Qualifications and Skills

• High School Diploma, GED or equivalent.
• One year of experience working with farmworkers preferred.
• Ability to speak, read and write in English and Spanish clearly and concisely.
• Abiliyt to communicate effectively with individuals and in small groups. 
• Current, valid driver's license.
• Ability to exercise flexibility in work schedule.
