Help deliver on their promise to tailor primary care to geriatric patients within Assisted Living and Memory Care facilities in and around
Kenosha, WI. Position Description - Full-time physicians care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day.
- Collaborate and provide clinical expertise with Nurse Practitioners and Physician Assistants.
- Manage a panel of 200+ patients (approximately 12-16 patients per/day)
- Flexibility in designing and managing your schedule
- Develop and implement care plans appropriate to patients and their desires
- Clinical support from clinical assistants, behavioral care managers, psych consultants, and care coordination through LPNs, RNs, and social workers.
- Access to diagnostics such as labs and x-rays as needed
- Collaborate with community partners within home care, hospice, and other resources as needed
