Job description

The Admissions RN responsible for all aspects related to the initial patient assessment and plan of care upon admission for patients across the life span (perinatal, pediatrics, and adults) according to established standards and practice. Responsible for providing information about services to potential patients, referral sources, and providing education to the community as needed. Responsible for skilled nursing assessment and hospice plan of care in order to maximize the comfort of patients and families consistent with The Elizabeth Hospice standards. Collaboratively works with the Interdisciplinary Team to ensure continuity of care for every patient.

 

The Elizabeth Hospice is San Diego’s oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Work Schedule: 12:00 p.m.-8:30 p.m. 5 days, days will vary. 

Salary: $45-$60 Hourly. Subject to experience and qualifications.

Regular Full-Time

Travel required for patient visits throughout San Diego and South Riverside County.

Occasional travel for educational purposes.

Weekend commitment: Every Other Weekend

 

  1. Performs patient visits as needed.
  2. Assists with discharges from facilities to the site of Hospice care.
  3. Performs Admissions at the site of Hospice Care.
  4. Assists the Hospital Liaison Team to meet the needs within the hospitals.
  5. Other Hospice nursing visits as assigned.
  6. Effectively manages time to meet department expectations in regards to admissions.
  7. Responsible for the direction of care provided by LVN and HA in accordance with patient’s plan of care.
  8. Responsible for the supervision of services provided by the LVN and HA per TEH policy and regulatory requirements.
  9. Assess and evaluate every patient to determine the appropriate plan of care (POC).
  10. Obtain relevant information and supporting documentation.
  11. Independently assess patient/family immediate care/symptom management needs in a timely manner.
  12. Consult with attending MD and other required medical personnel.
  13. Develop Admissions POC and communicate initial assessment, admission information, financial information, plan of care, and changes to the plan of care to the IDT on all assigned cases.
  14. Provides appropriate follow-up to outside case managers in regards to insurance and authorizations.
  15. Reviews borderline/complex cases per protocol. Adheres to the Non-Admit Policy
  16. Coordinate admission to hospice with other facilities, assisting with implementation and coordination of services.
  17. Effectively manages symptoms.
  18. Participates in training and mentoring of new staff.
  19. Keeps current with all department policies and procedures.
  20. Adhering to the E-way standards is a requirement in performing all job duties.
  21. Other job duties as assigned.
  1. Associate degree in nursing with 1-2 years acute care experience required.
  2. Certification as hospice nurse preferred.
  3. 1-2 years hospice experience preferred.
  4. Current CA Registered Nurse (RN) professional license.
  5. Demonstrated ability to work effectively with patients, physicians, community agencies and institutions. Interest and expertise in marketing preferred.
  6. Demonstrated ability to communicate effectively with members of the hospice interdisciplinary team.
  7. Proficient computer skills required.
  8. Demonstrated ability to multi-task and manage crisis situations.
  9. Current proof of CPR-BLS and negative TB test result.
  10. Current driver’s license with proof of insurance.
  11. Bi-lingual (Spanish/English) preferred.
