PURPOSE : The Dietician provides nutritional services and counseling to Grace clients and to their families as applicable in order to maximize the patient's nutritional status and improve clinical outcomes.
Job Description:
Conduct home or facility visits to assess the nutritional needs of hospice patients and to plan a suitable diet that is consistent with the patient's need.
Prepare a written plan of dietary care as part of the interdisciplinary plan of care.
Deliver counseling service to patients and family members, and provide instruction on how to prepare attractive and appropriate meals.
Provide in-service education to members of the patient care team.
Meet with the interdisciplinary team whenever acting as a consultant to the patient/family.
Provide outstanding customer service to all external and internal customers
Ensure that Grace Healthcare Services’ best practices are incorporated into all job activities/responsibilities.
Perform other duties as necessary.
Qualifications:
Registered Dietician license; Bachelor’s degree preferably from an accredited American Dietetic Association program
Minimum two years experience; one year in a community health agency with consultant and teaching responsibilities
Ability to travel within the State of NJ
