Job description

We are currently seeking a qualified Registered Nurse (RN) to join our dedicated team, assisting our patient population in navigating, preventing, and managing their health through continuous care programs, such as Remote Patient Monitoring and Chronic Care Management. Our team is committed to anticipating the needs of our patients and establishing personal connections to coach individuals towards their optimal health.

Responsibilities:

Welcome patients into continuous care programs, providing a comprehensive review of benefits and services.
Educate patients on the proper frequency and use of their assigned in-home monitoring devices.
Develop personalized, comprehensive care plans with patients through phone or video visits.
Identify and address any barriers hindering patient success.
Offer specific education and coaching on managing chronic conditions.
Maintain regular communication with patients to review readings and provide monthly updates on their care plans.
Act as the primary point of contact for non-emergent needs, including medication refills, scheduling, and appointment reminders.
Proactively anticipate and bridge any gaps in patient care.
Assist patients in navigating healthcare, aiding in tasks such as scheduling appointments, coordinating with specialists, and ensuring preventative screenings are completed.
Review and evaluate in-home device readings in real time during normal business hours.
Follow established protocols for identifying, communicating, and documenting device trends and associated symptoms in the patient's medical record.
Escalate concerning readings and/or symptoms to the provider following a communication protocol.
Establish a meaningful rapport that builds trust, open communication, and motivation for positive health changes.
Collaborate with virtual team members and in-office staff to ensure patient needs are met.
Requirements:

Active and Unrestricted Georgia RN License (Compact License preferred).
Minimum of three (3) years of experience in adult health preferred.
Background in adult chronic health conditions (Medical Surgical, Outpatient/Clinic, Care Management) preferred.
Proficient in various Electronic Medical Record (EMR) systems.
Strong knowledge, skills, and interest in basic computer proficiency.
Proven ability to problem-solve and multitask.
Excellent oral and written communication skills.
Strong teamwork skills.
Clean background check and drug screening.
Comfortable working remotely while maintaining collaborative engagement.
