The RN-Sepsis Coordinator is accountable for assessing and coordinating the management of potential septic patients or those who have been diagnosed with sepsis. Serving as a role model, provides clinical and professional guidance, and conducts needs assessments. Provides education programs that meet the needs of the patients, staff, and the organization. Serves as a professional practice and clinical expert at the bedside and in the classroom. The RN-Sepsis Coordinator ensures that septic patients receive appropriate care in a timely manner. May participate in independent and collaborative research activities that align with the mission/vision/values, identifying quality/performance improvement opportunities, and addressing staff development issues as needed throughout the organization.

License /Certification
Required: CA RN License

Education
Preferred: BSN

Experience
Required: Minimum of three years RN experience in acute care.

Preferred: Critical Care and/or Emergency Department experience.

Knowledge/Skills/Abilities

Effective communication skills including speaking, enunciating, and writing. Able to receive and effectively give information in person, by telephone, and in writing.

Intermediate knowledge of Microsoft Office Products, including Word, Excel, PowerPoint, and Outlook.

Detail-oriented and organized. Must have excellent follow-through.

Familiar with using data-driven methods to track and monitor patients on a daily basis.

Must have analytical skills for collecting and analyzing data.

Must thoroughly understand the Core Measure/CMS Sepsis Bundle requirements, and remain current on any changes or updates to either initiative.

Must be very comfortable communicating with physicians and other nursing staff.

Demonstrates advanced clinical skills, and independent judgment.

Essential

On a daily basis, identifies potential septic patients and initiates measures to comply with the sepsis core measures.

Educates staff nurses on sepsis initiatives and the importance of timely interventions.

Works with physicians and other health care providers to implement sepsis protocols in a timely manner.

Evaluates current clinical staff knowledge of sepsis protocols and based on findings, creates a robust program which includes education, monitoring, reporting, creating nursing order sets, and managing order sets through final approval processes.

Monitors the sepsis program for compliance. Presents recommendations based on findings; implements performance improvement plans to improve sepsis patient outcomes.

Advises hospital staff, medical staff and/or communities regarding opportunities and methodologies for improvement. Educates staff and details physicians regarding performance improvement processes/initiatives and patient safety.

Attends and/or leads appropriate clinical team meetings to report findings from the Sepsis program.

Communicates appropriately and clearly to physicians, staff and administrative team.

Performs concurrent and retrospective data review on Sepsis patients within designated timeframe and assists with data abstraction as needed. Regularly reports results and findings.

Collaborates with the Quality Department to ensure timely and accurate data collection for reporting purposes.

Accesses hospital databases and reporting applications as needed for data reporting.

Develops ongoing Sepsis educational classes/training, including annual competencies, new grad training and nursing orientation.

Stays abreast of all new requirements and/or clinical information regarding Sepsis. Regularly reviews the Sepsis core measure requirements and communicates any changes in requirements to appropriate committees.

Fosters a culture of continuous performance improvement and metrics driven outcomes.

Acts as a change agent to develop new processes, enhance existing workflow, and train clinical and medical staff in new processes.

Assures appropriate patient education and assists staff, as appropriate, in adhering to the professional and clinical standards.

Assists the staff in accommodating special patient needs by serving as a consultant and/or providing alternative materials or resources.

Facilitates bedside educational needs and care conferences on complex patients/families.

Provides regular feedback to the department of GME through the RRT Rotation Chief regarding resident performance as members of the health care team.

Pay Range
$84,198.40 - $126,297.60
