Description

The Housing Stability area of Hennepin County's Health and Human Services is seeking a Case Management Assistant to be part of the Homeless to Housing Program, a 30-person housing-focused case management team. This position will have caseloads and will be working with people staying in shelters, outside, and in encampments to connect them to permanent housing by using a housing-first approach, relationship building, creating housing goal plans, and breaking down housing barriers.

Location and hours:

This position is hybrid and will require working at the Hennepin County Government Center at 300 South 6th Street, Minneapolis, MN 55487 and at community sites within Hennepin County with direct contact with residents. Work hours are Monday through Friday, 8 a.m. – 4:30 p.m. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.
