Job Purpose : The purpose of the Nevada Care Connection (NVCC) Resource Navigator position is to provide services to seniors, all persons with a disability of any age, families, veterans, Deaf & Hard of Hearing and caregivers with information and assistance to a wide range of community resources; help inform and educate people about their options; assist in linking them to programs, services, and provide assistance with various types of applications. The position may require minimal travel to various Northern Nevada counties.
Key responsibilities and accountabilities:
- Serve as a visible and trusted source of information on the full range of community resources, information on long-term care options, referrals and awareness.
- Special efforts will be made to reach beneficiaries with disabilities of any age and seniors
- Gathers sufficient information to accurately perform resource and service navigation assessments and pre-determination of eligibility with consumers
- Provide information about programs, services (public and private) and other benefits, make referrals and, help consumers get connected to appropriate services.
- Help consumers identify and clarify issues, barriers and needs, explores needs beyond the presenting problem, looking at short- and long-term solutions and finding creative options and solutions
- Partner with the Family Resource Centers, Retired Seniors Volunteer Program (RSVP), senior centers and appropriate community partners to assist in increasing outreach efforts throughout northern Nevada
- Provide outreach and visits to offsite locations, when needed
- Outreach to service areas may include travel to: Elko, Eureka, Humboldt, Lander, and White Pine counties, when needed
- Participates in program development, marketing and outreach activities including representing NVCC at public information fairs.
- Work closely with NVCC leadership to ensure that any issues are handled efficiently.
- Implement critical thinking abilities with an emphasis on research skills and resourcefulness.
- Ability to be both compassionate and professional while serving our population.

- Must attend staff meetings and be willing to meet with manager on a consistent and regular basis
- Ability to adapt to changing work environment and duties
Other requirements :
- Bilingual preferred
- Minimal travel may be needed
- Ability to be both compassionate and professional while serving target populations
- Obtain Nevada MAP certification within 2 months of hire date
- Demonstrate experience in planning and execution of organization communication
- Ability to describe and communicate complex concepts
- Gain clear and working knowledge of social services available throughout our communities; establish and maintain appropriate working relationship with persons and agencies involved
- Attend community meetings as requested
QUALIFICATIONS:
- Able to drive for up to four hours at a time, when needed
- Minimum Education: High School Diploma; AA Degree Preferred
- Minimum 2 years' experience in social services, or non-profit work
- Strong verbal and communication skills
- Minimum two years computer experience including Microsoft operating systems, word processing, spreadsheets, data base input and report generation.
- Knowledge of social services programs preferred
- Excellent customer service skills
- Excellent communication skills, both oral and written
- Excellent organizational skills
- Be able to stand between 3 - 6 hours per day
- Be able to sit at a computer for up to 4 - 6 hours per day
- Must have a valid driver's license and be able to drive the AHN Company Car when needed
- Must be able to lift, carry up to 30 pounds with a fair consistency
- Required to pass a background check through the State of Nevada
