JOB DESCRIPTION: The Registered Nurse case manager is directly responsible to the Director of Nursing. The Registered nurse case manager is responsible for overall supervision of the care delivery to patient/family caseloads. The Registered nurse case manager is advised of other patient/families currently receiving Hospice care to assist with call coverage. The Registered nurse case manager acts as direct supervisor for certified nursing assistants, home makers and massage therapists.


RESPONSIBILITIES AND FUNCTIONS

A. Professional and clinical services:

1. Perform new patient intake assessments, including physical, psychological, social, and spiritual parameters. Patients are assigned by the DON or other hospice administrator.
2. Develops or assists with the development of a comprehensive plan of care for patients based on the comprehensive assessment.
3. Makes appropriate referrals to other community agencies as determined by patient/family needs and resources.
4. Coordinates care with the Hospice team, i.e., Hospice staff, primary physician, volunteers, spiritual care coordinator and patient/family; responsible for direction and appropriateness of referrals.
5. Supervises and/or provides direct patient care within the guidelines of registered nursing practice in the State of Montana and as defined by HOSPICE OF MISSOULA.
6. Provides bereavement follow up and/or referral within the guidelines of hospice's bereavement policy.
7. May share in 24 hour call coverage as assigned by the DON, or means developed by mutual team. Call coverage is for all active Hospice patient/families and new referrals received.
8. Makes referral and assists in the selection of hospice volunteers for specific patient/families. Introduces and supervises the volunteers, when necessary.
9. Attends volunteer support and educational meetings as schedule allows.

A. Educational and outreach:

1. The staff nurse will assist in public education presentations when appropriate.
2. The staff nurse will assist in volunteer training presentations as needed.
3. The staff nurse will participate in new employee orientation and training
4. The staff nurse will at times be expected to participate in community educational preceptor training programs

OTHER DUTIES

1. The registered nurse case manager will be responsible for ensuring completeness of their patient's medical records.
2. Ensure all documentation is up to date and complete in the patient's medical chart.
3. Office clinical nurse backup. RN case manager may serve as the clinical nurse backup in the stance that the DON is absent to offer additional support to both the office staff and clinical staff, accept new referrals and telephone service.
5. Other assignments as seen fit by Hospice DON, Administrator, Director or Unit manager.


QUALIFICATIONS

Physical Requirements

• Heavy Work: Sitting/standing, exerting up to 100 lbs. occasionally and/or 50 lbs. frequently, 20 lbs. constantly

The minimum requirements of this position require this individual to:

• Perform repetitive tasks and/or motions
• Distinguish colors
• Hear alarms/telephone/tape recorder/normal speaking voice
• Have good manual dexterity
• Have good eye-hand- foot coordination
• Have clarity of vision with or without corrective lenses at all times
• Interpersonal skills
• Competent home nursing care skills
• Skills in conflict resolution
• Interagency and referral skills and experience.
• Organizational skills

Other nonphysical requirements:

• Must have valid Current Driver's License
• Furnish own transportation
• Must maintain current automobile insurance coverage
• Graduate of an accredited Registered Nursing Program.
• Demonstrates knowledge of an ability to apply sound nursing principles to Hospice care.
• A MINIMUM of one year's experience in hospice, home health, or experiential equivalent preferred.
• Maintains current Registered Nurse license in the State of Montana.
• Demonstrates ability to supervise volunteers
• Ability to supervise peers
