Job description

Palliative Care (Hospice) NP or PA 

Join an expanding Palliative Care (Hospice) team. Outpatient clinic (Monday - Friday, 8a - 5p), no weekends, no call. Support 6 patients per day. Practice autonomously. Assess, plan, implement, and evaluate care needs. Symptom management, chronic disease management, goals of care, advanced care planning.

Requirements:

• BC Family Medicine or Internal Medicine
• Palliative Care (Hospice) desired
• Geriatric Certification required, or willing to obtain within 1 year
• OH license, or eligible
