Job description
The Clinical Transplant Coordinator (RN) works collaboratively within the multidisciplinary healthcare team and is responsible for providing comprehensive care to transplant patients, across the health care continuum from inpatient to outpatient. Serve as an integrated direct patient care nurse with the primary clinical responsibilities of teaching patients that have received a transplant, coordinating care for the patient from inpatient setting to outpatient setting. Tasks include comprehensive assessment to develop and implement treatment plans, monitor the effectiveness of clinical interventions, and provide on-going support, counselling, and education to meet patient needs. Incorporate quality assessment, performance improvement, clinical research and teaching to maximize and improve patient care activities. This role is primarily inpatient and will worth collaboratively with the inpatient transplant team and outpatient transplant team to establish a consistent model for transitions across the continuum of care.

Requirements
BSN, required
Master’s degree, preferred
Current licensure and registration (or valid permit) as a Registered Nurse with the New York State
A minimum of 4 years of clinical experience in a highly specialized complex care program.
Transplant experience preferred
Effective English communication skills, both written and oral
Ability to use data entry and retrieval system
Spanish speaking preferred
