Physical Therapist (PT)

Home Health

Hours: Monday to Friday /8am to 5pm

PRN

Compensation depends upon candidate’s years of experience and internal equity.

Base Rate: $40.00/Hourly (Gross)

PT Start of Care: $150.00

PT Evaluations/Discharge: $85.00

Reg/routine visit: $70.00 (weekend is same)

Case Conference Rate: $40.00

Mileage: per company policy

The Physical Therapist will be responsible for both evaluation and treatment of their patient caseload

• Current Texas State licensure or eligibility for state licensure as a Physical Therapist required
• Certified BLS or completion in first 90 days of employment required
• valid state Driver’s License
• Home Health Expirience
