The Home Health Clinical Manager-Preceptor Position

We are looking for a Full Time RN for the position of Home Health Clinical Manager for our Columbia office . This person will be responsible for managing, coordinating, and educating regarding appropriate home care services from admission to discharge. This position plans and coordinates the delivery of each assigned patient's plan of care in accordance with physician orders.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

• Assists with training new and established clinical staff to ensure quality care and compliance. This will include accompanying clinicians on patient visits, skilled nursing competency checkoffs, and OASIS education.
• Participates in the investigation and resolution of internal and external complaints.
• Coordinates all direct care patient services provided by clinical personnel.
• Prepares for and conducts weekly patient care conference.
• Instructs and guides clinicians to promote best practice in delivery of home care services and is consistently available during operating hours to assist clinicians as needed. (Normal operating hours are Monday through Friday 8 am - 4:30 pm)
• Collaborates with Director and QA staff to assess clinical education needs based on the review of documentation.
• Maintains ongoing clinical knowledge through accredited training programs. Provides interpretation of knowledge and direction of staff.
• Tracks infection control, potential avoidable events, and wound care trends.
• Review and approve physician orders.

WORK SETTING:

• Primarily works in office except for occasional in field clinician precepting.

QUALIFICATIONS for Home Health Clinical Manager-Preceptor:

• **CANDIDTES MUST HAVE HOME HEALTH AND OASIS EXPERIENCE**
• Current, valid, and unencumbered Tennessee RN license or Compact State licensure
• Current BLS/CPR
• Current and valid driver license, transportation, and auto insurance
• Two or more years home health experience required
• One or more years EMR experience required
• Must be computer proficient
• Must be self-motivated and customer service oriented
• Must be able to pass background and drug screening
