Meridian HealthCare in Mahoning County, OH is looking to hire a full-time Behavioral Health and Maternal Care Coordinator to oversee our MOMS Program.

A DAY IN THE LIFE OF A BEHAVIORAL HEALTH AND MATERNAL CARE COORDINATOR

• Work with OBGYN referrals to ensure prenatal care services
• Participates in the learning centers and teleconferencing meetings throughout the country
• Coordinates community resources, stakeholders, and medical professional meetings
• Triages and manages patients with medical and behavioral health conditions within the primary care setting
• Work with primary care providers regarding referrals to mental health and/or substance use treatment
• Assesses the clinical status of patients referred by the mental health providers through brief consultative meetings
• provides client access to individualized treatment to ensure client rights are upheld
• facilitate client's active participation in treatment planning whenever possible
• inform clients of alternative treatment services and procedures that are available
• Optimize client's accessibility to needed services
• Assures accurate documentation of services
• Conducts individual and group counseling and psychoeducational services to clients and/or families

QUALIFICATIONS FOR A BEHAVIORIAL HEALTH AND MATERNAL CARE COORDINATOR

• Registered Nurse or LPN, LSW, LPC or higher with Master's Degree required
