OPEN POSITION: 
Nurse Practitioner - Skilled Nursing Facility

SCHEDULE: 
- Full-Time
- 40 Hours per Week
- Monday through Friday
- Some Flexibility

COMPENSATION:
- $100,000 to $120,000 Base Salary, dependent upon experience

POSITION DESCRIPTION: 
Serve as a primary care provider for geriatric patients located in a skilled nursing facility.

The practice believes strongly in proactive medicine, with a clinical focus on:

- geriatric primary care
- preventative care
- chronic disease management
- medication management
- acute needs

This position will be located in one (1) skilled nursing facility, rounding on patients based on need and health maintenance visits.

The practice uses an EHR on a mobile-based platform and nurse practitioners are supported by dedicated team members who assist with orders, referrals, phone messages, etc.

Providers share on-call responsibilities, which is phone triage only and light volume.  Full-time providers are typically on call no more than 3 nights per month and some flexibility is offered.

REQUIREMENTS:
- Ohio NP license
- DEA
- 1+ year advanced practice experience
- previous experience in a relevant environment (geriatrics, LTC, etc.)
