CarepathRx transforms hospital pharmacy from a cost center into an active revenue generator through a powerful combination of technology, market-leading pharmacy services and wrap-around services.
Job Details: Responsibilities - As part of the sales team, quarterbacks onboarding process of patients from hospital or physician offices to home setting specific to patients in need of Home IV therapy or enteral nutrition.
- Includes commissions/bonus incentive program.
- Collaborates with the multidisciplinary team and servicing home health agencies to facilitate quality, cost effective services in the home, while maintaining continuity of care and achieving positive patient outcomes.
- Performs sales support in and around assigned hospitals, including all relevant clinics and physician offices.
- Coordinates entire referral, including insurance verification, home health scheduling, discussing financial responsibilities with patient, working closely with infusion pharmacy operational staff, etc.
- Performs basic patient education in relation to therapy.
- Gives high priority to patient and referral source satisfaction.
- Follows established administrative and human resources policies and procedures to ensure compliance at all times.
- Attends educational in services, departmental staff meetings, and mandatory training and UPMC Health System meetings as required.
- Maintains patient information and records confidentiality.
