CCBHC Registered Nurse

Summary The CCBHC Registered Nurse (RN) will provide primary care screening and monitoring of key health indicators and health risks (e.g., BMI, blood pressure, tobacco use, HIV/Viral Hepatitis). They will also document patient care, assist physicians, and collaborate with other members of the patient care team.

Key Responsibilities - Asses and Identify patient's need for care.
- Provide care according to the established care plan
- Provide nursing care and monitor care as per standards
- Document patient care to accurately reflect patient status and patient care activities
- Assist physicians in care of the patients
- Work collaboratively with other members of the patient care team to ensure optimal patient care
- Record nursing needs on care plan and update care plan to assure continuity of care
- Provide initial assessment and plan of care for home care clients
- Assist in training and reinforcement of care needed during a shift of care
- Provide a full range of nursing care to patients with complex care needs utilizing the nursing process
Qualifications and Experience - Bachelor degree or Master degree in Nursing (Bachelor's degree).
- Pass the National Council Licensure Examination or NCLEX-RN.
- Current state licensure as a registered nurse and BLS certification required.
- American Heart Association BLS (High school diploma)/CPR
- ACLS certification (Trades certificate / diploma)
- Ability to effectively communicate with patients, families, physicians and hospital staff.
- Basic computer skills (College diploma).
