Job description

We are currently seeking dedicated individuals to join our compassionate team as Social Workers. These positions offer immediate start upon acceptance and employment clearance, with initial funding through 6/30/2024 and potential for extension.

Compensation:

Pay Rate: $38.89/hr

Work Schedule:

Hybrid schedule: 3 days in the office, 2 days remote (Koreatown, Los Angeles)
Key Responsibilities:

Conduct psychosocial assessments and facilitate linkage to mental health and other social services for clients facing complex medical and psychosocial challenges, fostering their engagement in care.
Actively participate in outreach activities targeting highly vulnerable clients in non-clinical settings at risk of or experiencing syphilis.
Contribute insights to multi-disciplinary team case conferences, sharing findings and lessons learned to enhance client care plans and programmatic response.
Make referrals to, coordinate with, and utilize community resources, agencies, other professional disciplines, and County departments in alignment with patient treatment plans.
Develop and implement social work treatment plans based on analysis of the patient's psychosocial factors and medical conditions.
Minimum Qualifications:

Master's Degree in Social Work from a graduate school accredited by the Council on Social Work Education, including supervised clinical social work fieldwork experience.
One year of experience in medical, psychiatric, family, or child welfare social work involving issues of medical, mental, or emotional adjustment.
A valid, current license as a Licensed Clinical Social Worker issued by the California Board of Behavioral Sciences or registration as an Associate Clinical Social Worker within six (6) months of employment. Associate Clinical Social Workers must maintain good standing for the duration of applicable waivers permitted by law, not to exceed six years.
A valid California Class C Driver License or own reliable form of transportation.
Desirable Skill:

Knowledge of home visitation programs, Parents as Teacher, and/or Healthy Families of America.
