Responsibilities: * Skilled Nursing Care: Deliver a range of skilled nursing services, including wound care, medication management, IV therapy, and other specialized treatments, in the homes of our patients. * Patient Assessment: Conduct comprehensive assessments to determine patients' needs and develop individualized care plans tailored to promote optimal health outcomes. * Collaboration: Work closely with a multidisciplinary team, including physicians, therapists, and other healthcare professionals, to ensure coordinated and comprehensive care. * Patient Education: Empower patients and their families through education on managing health conditions, medications, and promoting overall wellness. * Documentation: Maintain accurate and thorough documentation of patient care, assessments, and interventions in accordance with regulatory standards.
