RESPONSIBILITIES:

As an RN with us, your primary responsibilities include conducting initial and subsequent clinical eligibility assessments for MassHealth consumers placed in nursing facilities. You will play a vital role as an educator and advocate, assisting consumers in identifying and obtaining necessary services. Collaboration is key, and you'll engage in case conferences with care managers, nursing facility staff, physicians, and other community professionals to ensure person-centered care planning.

QUALIFICATIONS:

Current, active, unrestricted license to practice as a Registered Nurse in the State of Massachusetts.
Bachelor of Science degree in Nursing, Associates of Science degree in Nursing, or Diploma RN.
Minimum of one year of clinical experience in the community or long-term care setting. (Diploma RNs require an additional 2 years of experience.)
Reliable car, current driver's license, and a safe driving record are essential.
