Job description

Nuvance Health has a network of convenient hospital and outpatient locations - Danbury Hospital, New Milford Hospital, Norwalk Hospital and Sharon Hospital in Connecticut, and Northern Dutchess Hospital, Putnam Hospital Center and Vassar Brothers Medical Center in New York - plus multiple primary and specialty care physician practices locations, including The Heart Center, a leading provider of cardiology care, and two urgent care offices. Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.

Summary

Following a medically prescribed physical therapy program, evaluates, treats and educates pediatric, adolescent, adult or geriatric patients who have disabilities, traumatic injuries, pain, disease or any dysfunction, and helps them reach their maximum performance and assume a place in their community while learning to live within the limits of his/her capabilities. Demonstrates knowledge and skills necessary to provide appropriate care to the age of the patient served in the inpatient and/or outpatient setting.

Provides direct patient care services to ensure the effectiveness of physical therapy programs.

Appropriate evaluates patients and establishes an effective treatment plan to achieve set goal

Handles consistently an assigned patient caseload and provides treatment to all patients within the assigned caseload.

Independently prioritizes caseload and available treatment times to provide for maximum efficiency.

Utilizes correct modalities of treatment and suggests variations.

Recognizes contraindications to continuing with scheduled treatment and consuls with responsible Physician.

Involves family or caregivers in treatment plan and discharge planning.

Respects patient privacy, confidentiality of patient information and exhibits behavior consistent with that of a professional health care provider.

Interprets the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs.

Provides the care needed as described in the Department's policies and procedures.

Demonstrates knowledge of physical therapy procedures and equipment allowing for safety and maximum patient benefits.

Attend all department related education activities and maintains knowledge base of all equipment and procedures.

Inspects all equipment before use to grantee patient safety.

Instructs patients adequately before each treatment session as to purpose and effects of treatment.

Assists in keeping track of inventory items required for smooth operation.

Attends continuing education seminars to maintain knowledge related to best practices.

Performs appropriate documentation to maintain the departmental standard.

Documents accurately, the progression and outcome following each patient treatment, along with the patient's reaction to session.

Documents accurately the information necessary to initiate charges for above treatment sessions.

Monitors circumstances surrounding patient treatment and documents occurrences which are not consistent with department policies/procedures.

Participates in communication with other involved hospital personnel to provide for quality patient care and cost-efficient programming.

Communicates efficiently and effectively on an ongoing basis with colleagues and patients.

Communicates to department administration any suggestions which may improve operations.

Provides confident instruction to patients and family members in preparation for discharge or for carry over of home exercise programs.

Performs other duties as outlined to improve the overall effectiveness of the department as both a treatment facility and an educational environment.

Helps maintain a safe and orderly department.

Strives to make good use of time; seeks out work which needs to be completed.

Demonstrates a complete understanding of hospital fire and safety procedures. Reviews department and employee human resource policies to maintain a complete understanding of hospital policies and procedures.

Promotes a safe and efficient working environment by adhering to hospital and department policies and procedures.

Adheres to and promotes Infection Control policies and procedures.

Adheres to safety regulations reporting any variance to the Director of Rehab immediately.

Patient relations - Demonstrates regard for the dignity and respect of all patients, family members, visitors and hospital personnel as defined in the philosophy of Health-Quest and Putnam Hospital Center.


Displays a caring and courteous attitude and respect for all patients, family members, visitors and hospital personnel as defined by the Mission and Vison of Health-Quest.

Promotes and contributes positively to intra-departmental and inter-departmental relationships. Maintains employee and departmental information confidentiality.

Adheres to department and hospital dress code wearing picture identification 100% of the time.

Education and Experience Requirements:

Graduated from an accredited physical therapy school with a B.S./M.S./DPT degree.

Minimum Knowledge, Skills and Abilities Requirements:

Excellent communication and problem-solving skills

Willingness to extend self as needed to meet the needs of the patient and department

License, Registration, or Certification Requirements:

Active New York State License or eligible with a temporary permit
