Summary of Primary Function
Delivers patient care utilizing the nursing process through data collection and assessment, identifies and determines the priority of patient’s problems/needs, and evaluates the process and outcome of nursing care; coordinates, provides and directs patient teaching activities and coordinates the care provided by health team members.
Essential Job Functions
In collaboration with the interdisciplinary care team, conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment and evaluates treatment effectiveness; administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures; serves as point of contact for patients' care coordination throughout hospital departments. Acts as a patient safety advocate by participating in ongoing quality improvement in the department. Incorporates the professional practice model into care delivery

Certifications
BLS Basic Life Support - American Heart Association (preferred)
Valid nursing license from the state of practice (required)
Education
Associate of Nursing, nursing (required)
OR
Diploma in Nursing (required)
Bachelors of Science, nursing (preferred)
Initiates and maintains communication with the hospice physician, client and family and interdisciplinary team members regarding assessment findings and plan of care. Instructs and educates patient and family regarding provision of care. Participates and assists with orientation and in-service training programs. Utilizes appropriate care standards to coordinate client care with documentation to support variances. Individualizes plan of care within care standard to effect maximal client outcomes. Implements discharge planning upon admission and coordinates. Consistently assesses client needs using appropriate integrates tools. Assists admit team in taking call for admission coverage and accepts other assignments as appropriate, Documents care in reimbursable terminology to include coding with timely/accurate submission of certifications and all orders. Complies with and is knowledgeable of hospital and department safety procedures and policies. Meets established productivity standards. Is knowledgeable and participates in hospice performance improvement program. Utilizes computer system for information and to complete tasks. Interacts with clients in a way that exemplifies the hospitals mission and core values. Attends annual in service, department meetings, and other training sessions relative to job. Possesses a valid driver’s license and auto insurance required. Must be willing to drive in inclement conditions
Lifting walking, sitting, standing and bending may be required. Driving required, use physical demands A/A nurse. Environment: outside and inside. Exposure to environmental factors, chemicals, solvents and bactericidal agents.
