Job description
The overall function of this job is to engage patient/family in an effective partnership with the clinical team in assessing, understanding and treating the patient and the family unit. This would include a focus on a discharge plan that would help the patient achieve improved functioning in the community and a reduced level of recidivism. The clinical therapist will interpret the clinical objectives of the team to the patient/family and represent and advocate for the patient/family to the team and to the community.
PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions)
- Prepares a comprehensive accurate summary of each patient/family contact all progress throughout hospitalization in the medical record, making appropriate post care recommendations and discharge dispositions.
- Provides a timely, comprehensive and accurate assessment of the psychosocial needs of the patient/family and recommends appropriate treatment interventions via verbal and written reports to team members and documents accurately in the chart.
- Provides patients/family with interventions for managing problematic behaviors/symptoms and coordinates discharge plans for the patient as it relates to environmental barriers that place restrictions on resource availability.
- Provides supportive family, group and educational intervention to an assigned caseload in a consistent manner.
JOB SPECIFICATIONS(Minimum Requirements)
KNOWLEDGE, SKILLS, AND ABILITIES
- LCSW preferred, LCMC accepted.
- Excellent written and verbal skills.
- Ability to problem solve and work independently and as a member of multidisciplinary team.
- Mature and flexible personality in meeting unanticipated demands and priorities.
- Ability to tolerate a wide range of emotional behavior and to work cooperatively in a multidisciplinary team.
- Understanding of human behavior especially that relating to psychiatric illness.
- Understanding family systems and dynamics in relation to mental illness.
- Knowing community resources.
EDUCATION - Masters Social Work Required
- Licensed Clinical Social Work
- Certified Cardiac Pulmonary Resuscitation
