Job description

The On-Call Clinical Case Manager Specialist oversees care coordination beyond regular business hours, guaranteeing a consistent level of customer service and clinical support around the clock. Reporting directly to the Vice President of Clinical Operations, this role requires dedication and a sense of urgency, maintaining the on-call phone for our designated markets during the weekend shift from Friday 4pm CT to Monday 8am CT.

Duties and Responsibilities:

Act as the primary point of contact for clinical and non-clinical inquiries during after-hours, fostering effective communication between clients, field staff, and office personnel.
Demonstrate a proactive approach in addressing issues that may arise after hours, ensuring timely follow-up with clients and Direct Caregivers.
Set clear and realistic expectations, consistently following up on timelines.
Maintain the On-Call log by documenting all client communication and activities received after hours, providing a comprehensive report to the team by the start of the next business day.
Communicate and escalate client and field staff issues to management in accordance with policies.
Enforce compliance with State and Federal regulations.
Coordinate the implementation of each patient's Plan of Care through effective communication.
Provide support, guidance, and education to patients, patient families, and caregivers to maintain optimal physical and emotional well-being.
Uphold the mission, ethics, and goals of the company, representing in a positive and professional manner.
Qualifications:
Requirements:

Current state license as a Registered Nurse, BSN preferred.
Minimum of two years of nursing experience in a Home Health or Acute setting preferred.
Strong leadership capabilities with supervisory and interpersonal skills.
Detail-oriented, flexible, with excellent organizational and time management skills.
Current CPR card and health certificate as applicable.
Ability to meet deadlines and demonstrate strong communication skills.

Salary: $ 65,000.00 75,000.00 Per Year
