Job description

We are currently accepting applications for the position of, Case Manager to work in the Promise Program, an outpatient substance use program that provides services for adult women. SUDC/ASUDC Preferred. Spanish Speaking Preferred.

Schedule: Monday - Thursday; Four (4) 10 hour shifts

Essential Functions: (Performs all or some of the following)

• Connects clients with community resources
• Facilitates goal setting
• Teaches coping skills
• Creates behavior contracts
• Coordinates services with other agencies
• Ensures clients' needs are being met
• Assists with the intake and assessment process
• Educates and assists clients in accessing Medications Assisted Treatment (MAT)
• Attends and participates in staff team meetings

Minimum Qualifications:

Case Manager I Minimum Qualifications: Graduation from a college or university with a Bachelor's degree in social work, and working towards SSW licensure; OR Graduation with a Bachelor's degree in any field and the applicant has completed coursework required by Division of Occupational and Professional Licensing to apply for SSW licensure per Utah Code 58-60-205: 1. Social welfare policy; 2. Human growth and development; and 3. Social work practice and methods, and working towards SSW licensure. SUDC and ASUDC applicants also accepted.

Case Manager II Minimum Qualifications: Graduation from a college or university with a Bachelor's degree AND Licensed as a Social Service Worker (SSW).

Preferred Qualifications:

• SUD-C/A SUD-C
• Spanish Speaker

Compensation: $20.03 - $28.01 per hour (depending on experience)

Unlicensed base wage range $20.03 - $20.74 hourly. Licensed base wage range $22.11 - $28.01 hourly.

Additional compensation available for Spanish speakers, after testing proficiency.

Employment at Wasatch Behavioral Health is contingent upon successful completion of a drug screen, motor vehicle record review and criminal background check. After hire you must complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing. Upon receipt of a conditional offer of employment in the position, you must provide an official copy of licensure or certification.
