We are now accepting applications for an Intake Coordinator, must be a registered Licensed Practical Nurse, available to work:

• Full-time

Compensation

• Starting wages $28-$36/hr. depending on experience

Intake Coordinator

• Coordinates all intakes, evaluates, and screens for appropriateness of admission to home care according to admission policy guidelines.
• Communicates in a positive manner with referral sources including physicians, families, social workers, discharge planners, and other sources to establish good rapport and appropriate level of care.
• Demonstrate ability to evaluate and assess clients' needs with strong knowledge of continuity of care.
• Maintains client confidentiality. Abides by all HIPAA regulations.
• Determines eligibility for services and reimbursement options. Identifies and verifies insurance coverage for home care services and supplies. Obtains prior authorizations and on-going authorizations as needed.
• Coordinates intake referral process for admission of client to the agency that includes essential background information, hospital course, and history of present illness.
• Provides liaison between the agency and insurance payer source for case management coordination.
• Demonstrates the ability to provide strong customer service in a proactive manner including the ability to creatively problem-solve to facilitate the referral process and client care planning for admission to the agency.
• Maintains effective documentation of all communication with staff and payers to insure care coordination, case management, and to facilitate reimbursement.
• Provides on-going communication regarding reimbursement/payment issues to case manager and billing department.
• Collects statistical data on all referrals.
• Participates in agency client care conferences and in-services on an as-needed basis.
• Educates staff/clients/families/physicians and other sources regarding the utilization of home care services.
• All other duties as assigned.

Qualifications

• Licensed in the state of Colorado or multi-state license for Licensed Practical Nursing
• 2 years experience medical, surgical or acute care setting
• 2 years experience in home health care experience preferred
• Must have valid driver's license

Wage information is provided in compliance with the Colorado Equal Pay for Equal Work Act. The compensation offered to the successful applicant may vary based on factors including experience, skills, education, location, or other position-related reasons.
