Job description
Enhanced Care Management (ECM) is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of high-need youth and families through the coordination of services and comprehensive care management. The Lead Care Manager is the point person for youth and families who are in need of care coordination and linkages in the community.

- Engage in outreach activities to enroll new clients in ECM.
- Working with youth and families and their care team to conduct a comprehensive assessment and develop and update a care plan for each member.
- Organiz e care activities and maintain regular contact with outside providers to ensure coordination, including county substance use disorder and specialty mental health providers as appropriate .
- Manag e referrals, coordination, and follow-up to needed services and supports.
- Provid e education and identify support needs for youth and their family or other caregivers.
- Provid e services in person and accompany clients to appointments when needed.
- Engage in community outreach and identify supports and resources in the community , including building relationships with community providers .
- Complete required documentation in a timely manner .
- Attend supervision and other team meetings as required .
- At least three years of relevant work experience
- High school diploma or GED required
- Prior case management and community-based experience preferred
- Bilingual in Spanish a plus!
- A commitment to advancing racial equity at the organizational level.
- Strong written and verbal skills
- Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks.
- Fingerprint clearance
- Ability to work Monday- Fri d ay 9 am-5pm with flexibility to work some earlier mornings or evenings.
- Valid California Driver’s License
- This is a hybrid role
- Typical schedule is Monday - Friday 9:00 AM - 5:00 PM
- Some evening flexibility required
- Starting range: $21.63 - $24.13 per hour
- Actual salary dependent on creditable experience above the minimum qualification
- Additional compensation provided upon passing bilingual language proficiency exam
- Salary increases each year
