Job description
Responsible for working closely with hospitals and other facilities to coordinate continued patient care with home care healthcare services after discharge. Assures quality customer service to patients, families, referral sources and co-workers. Serves as a consultant, educator, and customer service specialist to referral sources.
Principal Accountabilities:
1. Develops initial plan of care for a Home health admission to Atlantic Visiting Nurse.
2. Coordinates with patients, families, physicians, and discharge planning personnel by obtaining complete clinical information’s for the home care admission.
3. Maintains current knowledge of clinical issues, regulatory and reimbursement standards, insurance contracts and negotiated rate amounts. Acts as a resource to patients, families, physicians, and discharge planners.
4. Performs other related duties as assigned.
Required:
Bachelor’s degree preferred in Health-related field.
Preferred:
1. Experience in Home Care or relevant area a plus.
