Job description

Under general supervision, this position is responsible for performing the physician suspension process; assisting Medical Staff in the completion of records; facilitating the hospital staff transcription process; answering and processing telephone requests; and responsible for the quality and completeness of the patient’s medical record. Full understanding of Medical Staff Rules and Regulations and chart completion requirements.

 

 

 

Coordinates physician completion with management of the suspension process.

1. Assists physician with record completion with customer satisfaction

2. Update EMR during completion process as soon as possible

3. Reinstate suspended physicians ASAP after chart completion

4. Assist with physician training on the EMR

5. Follows MS Rules and Regulations and processes for Suspension

6. Timely completion of the suspension process and adhere to suspension dates stipulated in the Medical Record Delinquency policy and procedure and Medical Staff Rules and Regulations

7. Sends updated suspension list timely

8. Performs monthly audit of incomplete charts and reports to the department Director

9. Accurately clears charts that have been completed by physicians

10. Finds any missing reports (including transcribed reports) for physician completion

11. Track and report Operative and History & Physical delinquency/suspension

12. Maintains physician logs, stats and OPPE (Medical Staff) reports for physician activity requests

13. Files charts back after completion in the appropriate files

 

C. Physician notifications

1. Process accurate physician notifications for delinquent documentation with 100% accuracy rate

2. Phone/fax requests are handled promptly before shift is complete

3. Responds to physicians’ inquiries regarding record documentation requirements promptly and before your shift is complete

4. Generates cumulative days on suspension report

5. Maintain documentation (computer and/or hard copy of physician delinquent and suspension process information for 2 years)

 

D. Attends department specific education/training, in-service’s, and staff meetings

1. Attends mandatory in-service’s/educational/training activities

2. Submits all required paperwork on time

3. Verifies, by signature/initials, attendance at staff meetings or reading of staff meeting minutes

 

E. Department specific performance improvement project.

1. Actively assists in performance improvement monitoring with VO/TO Orders.

2. Demonstrates understanding of performance improvement principles in job performance.

3. Serves as the liaison for Peer Review between the physicians and the Quality Department

4. Assists the Medical Staff Office with physician proctoring

 

F. Accurately files charts, records and documents utilizing terminal digit, alphabetic and numeric filing systems within 

department timeliness. Accurately checks returned charts into the system, and returns the charts to the appropriate

location.

 

G. Pulls requested medical records within time specified and accurately abstracts information into chart locator system.

 

H. Demonstrates proficiency in medical record documentation requirements, The Joint Commission, Title 22 and Conditions of Participation requirements. 

 

I. Enter deficiencies into the computer system ensuring the correct physicians are selected and the correct patient /visit

are selected, print the “lack” sheet, use color tags to identify pages for physician completion. Call physician offices for

missing pre-natal’s or other missing documentation. 

 

J. Provides assistance to the Coding function to assure coding queries are completed by the physician.

 

K. Assists in the orientation, training and development of other Medical Records clerical employees.

 

L. Other duties may be assigned as needed.

 

High School Degree 

This position requires the full understanding and active participation in fulfilling the mission of AHMC Anaheim Regional Medical Center (“ARMC”).
