POSITION TITLE: Community Health Navigator STATUS:Temporary GRADE: 1

DIV: Safety and Wellness PROGRAM: Health Promotion and Navigation

REPORTS TO: Senior Community Health Navigator SUPERVISES: N/A SALARY: $22/per hour

SUMMARY: The Community Health Navigator supports the social, economic and behavior health needs of disvested populations/communities. The Community Health Navigator works on an inter-disciplinary care team supporting members who are experiencing barriers affecting their health and access to health care. The Community Health Navigator (CHN) works to inform and empower through direct education, resource coordination, and encouragement to community members and their families to community resources. This position is temporary, with employment anticipated to end December 31, 2024.

COLLABORATES WITH: Collaborates with various YWCA Teams, clients and members, volunteers and interns and community partners.

ESSENTIAL RESPONSIBILITIES:

Direct Services: Build, cultivate, and deepen trusted relationships and partnerships with business, organizations, community groups and individuals to increase knowledge and reduce hesitancy with obtaining the health screenings. Promote everyday preventive actions, provide information to break down myths and rumors, and identify populations at risk. Establishes close relationships with and serves as primary point of contact for community residents seeking accurate information, support with registration/appointments or resources needed to access healthcare. Identify resources for clients to overcome barriers to care, such as generalized fear/mistrust of healthcare system, transportation, food/housing insecurity, immigration status, language, and childcare arrangements. Facilitate clients’ connection with regular primary care provider for ongoing preventive primary care and follow-up. Utilize social media platforms to affect awareness and availability of community resources. Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance and knowledgeable of Centers for Disease Control and Illinois Dept. of Public Health. Maintain client confidentiality according to HIPAA, YWCA Confidentiality Standard and other funder regulations.

Administrative Duties: Participate in routine team meetings and trainings related to CHN position. Work with Case Manager and partners to develop informational outreach material, curriculum or workshops/special events. Complete assigned data entry and other paperwork in accordance with timelines. Regularly check e-mail and voicemail and respond promptly and professionally to all inquiries. Accurately and efficiently, perform a variety of clerical duties including opening and processing mail, copying, filing, and preparing correspondence.

NON-ESSENTIAL RESPONSIBILITIES: Assist in the planning and implementation of special events and/or programs. May be required or invited to participate in community events.

OTHER DUTIES AS ASSIGNED: Other duties outside of the responsibilities of the position may be assigned, whether for a project, special needs task or other assignment.

QUALIFICATIONS: One year of community or customer service experience is required. A current driver’s license, verifiable good driving record and reliable transportation required. Verifiable connection with a trusted community group, organization or institution. Preference for bilingual/multilingual (English and Spanish) written and spoken. The position requires prior experience with Microsoft Office Suite - Word, Excel, Teams and Outlook. Demonstrated public speaking, writing and presentation skills with well-developed organizational and problem solving skills. Self-starter with strong time management skills.

Demonstrated skills for the position relate to collaboration, communication and skilled public speaking abilities; prior experience in community engagement is advantageous. The CHN must have the ability to diplomatically present information and work effectively with the public; skills in cultural competency are necessary. Team collaborations require the demonstrated ability to interact with other members of the work group.

The CHN must possess the ability to effectively manage shifting priorities and respond effectively to crises and changing circumstances. The CHN must possess a high tolerance for chaos and high-stress situations, with a service orientation.

*Note: A bilingual Community Health Navigator who works with a foreign language speaking community must demonstrate cultural competency with that community and fluency in both English and the community’s language (oral and written).

EDUCATION REQUIRED: AHigh School diploma, or GED, is required. Must have previous experience with community work. (i.e. Advocacy, volunteer, etc.)

WORK ENVIRONMENT: The CHN must be available for a flexible schedule, which includes on-call shifts, during workdays and weekends and travel locally to provide services to members. The work environment includes offices and other community locations depending on events. The noise level is usually moderate. movement requirements include walking, stooping or kneeling. There may be a need to reach or to lift objects, such as packages or supplies. Communication needs include seeing, hearing and speaking. There will be periods of sitting, holding a telephone and using computers. Work will be completed with individuals, small and large groups. WORK SCHEDULE: Monday – Friday, 8:30am-4:30pm. This position may require work on some evenings and weekends. Schedule can vary depending on the needs of the agency.
