SEMRHI is a network of Community Health Centers that has been serving the needs of Southeast Mississippi for almost 45 years.
SEMRHI has a nationally recognized Ryan White Program that serves the health care needs of people with HIV. It is the 2nd largest HIV provider in the state. A multidisciplinary team of internal medicine and infectious disease physicians, nurses, RN Case Managers, licensed master social workers (LMSWs) and licensed clinical social worker (LCSW) provides a team approach to meet primary, HIV and behavioral health care and services for people with HIV.
Under the direction of the Director of Social Services, the Ryan White RN Case Manager is responsible for providing case management services for the Ryan White Program, Early Intervention and Services Program. The responsibilities of the RN Case Manager include providing counseling, educa tion, and service coordination to patients eligible for the program and assisting in all data collection and reporting. The RN Case Manager is a community spokesperson for the Ryan White Program (RWP) and works with other agencies to enhance a system of care.
Job Responsibilities:
- Participates actively in Performance Improvement activities and monitors problems discovered
- Works closely with the clinic providers to investigate contributing factors in a patient's illness for continuity of care and optimal health outcomes
- Attends health fairs and/or outreach activities to distribute HIV/AIDS educational materials and to create an awareness of the RWP services
- Attends meetings and/or trainings for the Ryan White Program, the SEAETC trainings and other work-related meetings and trainings.
- Reports accurate and appropriate data at least monthly to the RWP Coordinator and/or CQI RN regarding the Ryan White Program
- Actively participates in the meetings with the Ryan White staff and the social services staff to ensure coordination
- Demonstrates positive personal attributes such as honesty, integrity and professionalism in all aspects of the job
- Demonstrates cultural competence in all activities and ensures that all patients are fairly treated with high quality care
- Demonstrates a clear and ongoing understanding of patient confidentiality and privacy standards
- Oversees effectively an assigned caseload of patients including the scheduling of clinic and/or specialty care visits, labs, screenings, vaccines, medication needs, transportation assistance, and maintains current RWP case management paperwork.
- Maintains accurate data files regarding assigned patients including EHR documentation and CAREWare database entries
- Completes data entry for all RWP funded services, labs and screening labs results, screenings, HIV medications, immunizations, referrals, and other required data into CAREWare database for assigned patients and as needed
- Completes internal and external referrals for patients when medically indicated, follows up on the outcome of the referrals and documents it in EHR and/or CAREWare.
- Completes ADAP applications for new enrollments, annual updates and enrollment changes on patients as needed in a timely manner.
- Completes and documents Rapid HIV testing per policy and procedures and accurately completes SeMRHI and MSDH Rapid HIV testing documents.
- Performs and documents case management assessment and acuity scale, mental health and substance abuse screenings, nutritional screening, adherence counseling, HIV education, and self-management skills for HIV+ patients on enrollment, annually and/or medically indicated and completes and documents an individualized care plan on enrollment and every six months.
- Generates an encounter for all face-to-face case management visits and/or Rapid HIV tests on the date of service.
- Documents face-to-face, telephone, Rapid HIV testing, and other patient-related encounters in the patient's electronic health record and/or CAREWare within 24 hours of the date of service.
- Maintains prompt and regular attendance.
- Verify timecard at the end of each pay period.
- Performs related work duties as assigned by supervisor.
Job Requirements:
- Bachelor's or associate degree in Nursing
- Must have knowledge of social service and health care delivery systems.
- Must have at least one year of nursing experience and/or prior work experience with People Living with HIV (PWH) in a community health, hospital, or health education related setting
- Valid license as a Registered Nurse in Mississippi required.
- Current Basic Life Support (BLS) through American Heart Association.
- Must be able to continuously use sound communication and counseling skills in handling title case management activities.
- Must be able to reason in planning and reporting.
- Must be able to communicate effectively both orally and in writing.
- Must be able to deal effectively with patients and staff.
Physical and Other Requirements:
- Must be able to pass a criminal background check, drug screen and physical assessment.
- Must be willing to do local and out-of-state travel.
- Must be able to sit, stand, or walk for long periods of time.
- Must be able to lift a minimum of 10 pounds routinely.
