Job description

MORES is a team-based crisis intervention for children and adolescents ages 3 to 21 years experiencing escalating emotional and/or behavioral needs. MORES provides up to four weeks of follow-up services for individuals who have experienced a behavioral health crisis. The goal of the MORES program is to stabilize the present concern, empower & educate families to monitor, manage & cope with similar situations in the future, establish & build resiliency skills and strengthen the natural support systems of families.  

Mobile Crisis Management – MORES Family Support Partner

 

Reports to: MORES Team Lead

 

Purpose: Assure maintenance of all minimum standards of Mobile Outreach, Response, Engagement and Stabilization (MORES) Team

 

Education/Experience/Other Requirements:  

 

  1. Have a Bachelor’s degree in a human services field from an accredited university and one year of experience working with the target population; or an associate degree in a human services field from an accredited school and two years of experience working with children/adolescents/transition-age youth; or a high school diploma or GED and a minimum of four years of experience working with children, adolescents, or transition-age youth
  2. Must hold or is in process to hold a National Certification in Family Partner.  
  3. Must have lived experience as a primary caregiver for a child who has/had mental health, substance use disorders, or intellectual/development disability. Must have experience in navigating any of the child and family -serving systems and teaching the family members who are involved.
  4. Must have 8 full years of driving experience and a valid driver's license . The following must be true in regards to DMV record:
    • No one with a past DWI on their record will ever be able to drive a company vehicle or transport a resident in a personal vehicle
    • No one under the age of 25 can drive a company vehicle
    • To transport residents in a personal vehicle you must have had a valid driver’s license for three years and a clean driving record. If you get your first license at the age of 16 you must be 20 and have nothing on your driving record. If you are 17 when you get your license then you must be 21 and have a clean driving record.
  5. No substantiated findings on the NC Personnel Registry/Health Care Program and must not be listed on the HHS/OIG List of Excluded Individual/Entities.
  6. SBI and DMV records must meet the requirements of area mental health contracts.
  7. Meet state personnel statutes and rule requirements for employment and training of individuals delivering MH and/or SA services. Family Support Partner will be supervised according to the rules of 10A NCAC 27 G.0104.
  8. Must be flexible and willing to work various hours including nights and weekends.
  9. Must report and communicate directly to the MORES Team Lead.

Responsibilities Regarding Consumer Rights:

 

  1. To practice and promote the policies and procedures of Therapeutic Alternatives, Inc., thereby ensuring basic rights to all people receiving our services.
  2. To attend meetings and in-services as directed by my supervisor for the purpose of continuing education regarding consumer rights, consumer choices, and consumer outcomes.
  3. To respect and maintain the confidentiality of all consumer information to which I am privileged during the carrying out of my general responsibilities.
  4. To be polite and courteous to all consumers, guests, and staff of this corporation.
  5. To learn/understand my role in the process and meaning of accreditation.

  1. Document the service provided in progress notes. The basic data included in these notes will include observations, consumer participation and progress, behavior changes or any pertinent information relating to the consumer. All documentation must be completed on the day the service is provided and completed electronically or in BLACK INK only. Use of “white-out” is not acceptable.

 

  1. Adhere to company policy that prohibits leaving consumer unattended in a vehicle or any other public place.

 

  1. Provide transportation for the consumer to community services/activities when required.

 

  1. Monitor progress notes, charts, programming materials. Maintain all required records.

 

  1. Submit all required documentation by the deadlines specified by the MORES Team Lead.

 

  1. Attend all mandatory staff meetings and consumer conferences or give advance notice to the MORES Team Lead as to reasons for inability to attend.

 

  1. Receive supervision by the MORES Team Lead.

 

  1. Report any changes in consumer’s condition, family situation, or needs to the appropriate personnel.

 

  1. Report any accident or incident to the MORES Team Lead.

 

  1. Report to MORES Team Lead any suspected abuse, neglect or exploitation of recipients.

 

  1. Be familiar with TA, Inc. policies and procedures including the securing of emergency services and the regulations concerning medication administration.

 

  1. Be familiar with HIPAA regulations and abide by them.

 

  1. Assure consumer’s rights are met and maintain consumer confidentiality at all times.

 

  1. Operate office equipment as necessary, i.e. calculator, copier, computer, fax, etc.

 

  1. Obtain and keep current all training required by the agency. Understand that failure to renew classes by their due date will result in being removed from the schedule.

 

  1. Perform other duties as required by circumstances.
