In this role, you will:

Provide comprehensive care to adult patients with complex chronic conditions in their homes and through telehealth.
Develop personalized care plans in collaboration with a supportive team.
Conduct thorough assessments, diagnose and manage various conditions, and order necessary tests.
Build trusting relationships with patients and their families, fostering patient engagement and empowerment.
Collaborate effectively with other healthcare professionals to ensure seamless care coordination.
Contribute to a fast-paced, high-energy environment focused on continuous improvement and innovation.
We are looking for someone who:

Possesses a strong clinical background and enjoys caring for patients like family.
Demonstrates deep empathy and a commitment to going the extra mile for patients.

Is a highly motivated team player with a can-do attitude.
Holds an unrestricted and active APRN license, current certification, and BLS certification.
Has 3+ years of experience as an NP, preferably in primary care, house calls, or related fields.
