Job description
- Job Title: PT Home Health Therapist
- Job Type:13 week contract
- Shift: Day 5x8-Hour (08:00 - 16:00)
- Start Date: ASAP
- Location: Cookeville, TN 38501
We are looking for a skilled and compassionate PT Home Health Therapist to join our team. As a Home Health Therapist, you will work with patients in their homes to provide rehabilitation and therapy services.
Responsibilities:
- Provide in-home physical therapy services to patients as prescribed by a physician.
- Develop and implement patient care plans that are individualized and goal-oriented.
- Evaluate and document patient progress and adjust treatment plans accordingly.
- Communicate regularly with physicians, nurses, and other healthcare professionals to coordinate patient care.
- Educate patients and their families on exercise programs, injury prevention, and proper body mechanics.
Requirements:
- Active state PT License
- Experience with Home Care Home Base and Oasis documentation
- At least 1-2 years of previous adult Home Health experience
- Must meet 30 productivity points per week
- COVID-19 vaccination required (Medical/Religious Exemptions Only)
- Reliable transportation and valid driver's license
- Ability to work independently and manage time effectively
Preferred Qualifications:
- Experience with a variety of patient diagnoses, including neurological, orthopedic, and cardiopulmonary.
- Strong communication and interpersonal skills
- Experience with EMR systems • Bilingual (English/Spanish) a plus
