Job Title: Physical Therapist - Home Health

Job Description:

We are currently seeking a skilled and experienced Physical Therapist to work in the Home Health setting. This is a 13-week renewable contract position with the possibility of an extension. The ideal candidate will possess a minimum of 1-2 years of PT Home Health experience and proficiency in HCHB charting.

Shift: Day 5x8-Hour (08:00 - 16:00)

Job Requirements:

• Must have a minimum of 1-2 years of PT Home Health experience and proficiency in HCHB charting.
• Must be able to complete all elements of Oasis, including admissions, recerts, rocs, discharges, and evals.
• Current PT (TN) License in hand and BLS certification.
• Travel to Cookeville/Sparta TN area.
• Mileage reimbursed at IRS rates will accept medical COVID exemptions.
• COVID-19 vaccination status required / Booster status required.
