REGISTERED NURSE CASE MANAGER- Chronic Care

In this role you will:

• Assure that Chronic Care patients are scheduled for follow up visits and receive phone calls periodically to support their health maintenance activities.
• Develop and utilize patient case registry and data collection program to track, record and summarize patient information for DPH staff.

• Triage patient telephone calls to team providers and identify need for urgent care and make appropriate referrals required for the management of patient care.
• Establish contact with patients enrolled in the Chronic Care Self-Management Program.
• Participate in development of unit goals, objectives, protocols, and procedures.
• Utilize those skills to interact with patients/clients, team members and staff from other departments.

The successful applicant will offer:

• Graduate of an accredited nursing program.
• RN with current Mass. Licensure
• Case Management experience preferred.
• Bilingual in Spanish/Khmer/Russian beneficial
• A desire to work as a collaborative member of a multi-disciplinary team.
