Highland Rivers Health is seeking Behavioral Health Assessors to be based in the Hamilton Medical Center Emergency Department.
Responsibilities include: - The assessor will perform behavioral health assessments in the Emergency Department and on the medical floors and will consult with physicians and staff, collateral contacts and community partners in order to determine the appropriate treatment disposition while ensuring the individual's safety.
- Conducts and interprets comprehensive, face to face or tele-medicine based assessments in an effort to make level of care recommendations.
- Facilitate access to the identified inpatient, residential or community based service for a diverse population.
- Review the intake criteria and scope of services for the level of care recommendation with the individual/guardian to ensure comprehension and improve clinical outcomes
- Act as a liaison between the individual and the determined level of care to include referral via BHL, faxing clinical information to receiving facilities and scheduling outpatient appointments as appropriate.
- Complete other duties such as phone based discharge follow up calls, phone based engagement activities, treatment/disposition planning dependent upon education, experience, and skills as needed.
- Record data for each assessment completed including but not limited to: presenting problem, order time, notified time, acknowledgement time, assessment date, assessment time, acceptance date, discharge date, payor source, disposition type, and disposition location.
- Complete other duties as assigned by the Director of Community Crisis or ER Manager.
Requirements Minimum: - Must be a LPC, LCSW, LMFT, APC, LMSW, or LAMFT or RN (BSN preferred but not required).
- Master's Degree in Social Work or Counseling.
- Must be able to pass a background check and drug screen
- Must stay current with all hospital, agency, and state training requirements
- Must complete the Mindset Training and stay current in crisis de-escalation intervention
Preferred: - At least 1 year doing comprehensive behavioral health assessments.
- Knowledge of community resources
- Strong written and verbal communication skills
- Bi-lingual abilities are an asset
