Greene County Health Care is a Community Health Center providing comprehensive medical, dental and behavioral health services for members of our communities in Greene, Pitt and Pamlico counties in eastern North Carolina.
Job Summary
The Case Manager serves Greene County Health Care’s patients by coordinating the management of patient-centered, cost effective care to specifically identified patient populations (i.e., patients with history of noncompliance with prescribed treatment regimen and frequent use of emergent care facilities and/or readmissions to acute care facilities.) Accesses, analyzes, customizes, coordinates and communicates the patient’s plan of care and collaborates with providers and other members of the multi-disciplinary health care team.
Responsibilities and Duties
- Assists patients to navigate through the healthcare system by acting as a patient advocate and facilitates patient education and access to healthcare and community resources.
- Coordinates continuity of patient care with external healthcare organizations and facilities, including the following transitions of care: hospital admission, discharge, and referrals from the primary care provider to specialty care providers.
- Promotes clear communication among clinical provider staff by ensuring consensus and updates regarding patient care plans.
- Functions as a liaison to external agencies and shares information to others which may impact care and/or services of patients.
- Actively participates in staff meetings, case conferences, and other meetings as appropriate.
- Promotes and role models collaborative practice relationships with all health care professionals.
- Recognizes the role that cultural diversity of patients and their families, employees, health care staff, and community members plays in achieving productive and positive relationships.
- Monitors patient conditions monthly or more frequently as indicated and creates an evidenced-based comprehensive action plan for each patient.
- Manages high-risk patient care, including management of patients with multiple co-morbidities and/or high risk for hospital readmission.
Qualifications and Skills
- Bachelor’s degree in Nursing (with a current RN license in good standing with the NC Board of Nursing), or a Bachelor’s degree in Social Work, Sociology, Psychology or other related field. Master’s degree in Nursing, Social Work, Sociology, Psychology or other related field preferred.
- Two (2) or more years of experience in case management.
- Experience assisting acute/chronically ill patients navigate the healthcare system preferred.
