• Treat chronic and acute conditions while caring for adults and children
• Take advantage of robust support from a team that includes two physicians, a nurse practitioner, two RNs, three office assistants, five medical assistants
• Practice model includes on site access to pulmonary medicine, care coordinator, and behavioral health consultant
• Participate in 1:6-week call schedule
