Position Overview:

As a Case Manager at the Framily Support Network, you will be instrumental in delivering our case management services under the guidance of the Director of Case Management. You will be responsible for creating personalized case plans for each client, ensuring all milestones are met. Your role is crucial in supporting our clients through their journey to stability and self-sufficiency, making you a backbone of our mission.

Key Responsibilities:

• Conduct comprehensive assessments of clients' needs to develop individualized case plans.
• Coordinate and monitor services for clients, including housing, employment, healthcare, and other essential supports.
• Regularly review case plans with clients, adjusting strategies as necessary to achieve milestones.
• Collaborate with other service providers and community partners to enhance support for clients.
• Maintain detailed records of clients’ progress and services provided.
• Participate in team meetings and contribute to the continuous improvement of our case management services.

Required Qualifications:

• Bachelor’s degree in Social Work, Psychology, or a related field.
• A minimum of 3 years of experience in case management or a similar role within a non-profit, social service, or healthcare setting.
• Strong understanding of the issues surrounding homelessness and a demonstrated passion for social service work.
• Excellent organizational, communication, and interpersonal skills, with the ability to manage multiple cases efficiently.
• Ability to work collaboratively in a team and independently with minimal supervision.
• Proficiency in data entry and case management software.
• Adaptability and flexibility to work in a dynamic and changing environment.
