Voyage Healthcare is looking for a full time
Anticoagulation Clinic (ACC) Registered Nurse (RN). Travel is required to our Crystal, Osseo and Maple Grove locations.The schedule is Monday through Friday, between the hours of 8:00 a.m. - 5:00 p.m. No evenings or weekends!
The ACC RN is responsible for performing point of care testing, monitor anticoagulant medication doses and provide patient education. This position may have the opportunity to be cross-trained into other clinical duties within the clinic as the need arises, i.e. administration of vaccines.
Responsibilities: - Responsible for planning and coordinating the daily activities.
- Responsible for maintaining patients INR values in ordered range by adjusting anticoagulation medication according to provider established protocol.
- Documentation of results, dosing and any other pertinent information affecting patient’s anticoagulation therapy.
- Determine appropriate medication dosage for patients based on lab results by utilizing physician established protocol in conjunction with nursing judgment.
- Patient teaching regarding anticoagulation therapy, medication changes, and dates of next office visit or lab appointment.
- Monitors for patients who have missed Anticoagulation Clinic appointments or who are overdue.
- Describes or explains what can and will be done for the patient, i.e., speak with provider, schedule same-day appointment, prescribe medication based on protocol
- Coordinates patient care by ensuring appointments are scheduled based on urgency of phone call, following established clinic protocols.
- Communicates patient changes or problems to providers.
- Maintains patient confidentiality and adheres to HIPAA regulations.
Requirements: - Graduate from an accredited school of nursing required.
- One year of nursing experience preferred.
- Be able to demonstrates effective communication skills with patients, families, providers, other staff, and outside health care professionals.
- Currently licensed in Minnesota with renewal of license as required by state law.
- This position requires strong communication skills, both written and verbal.
- Knowledge of nursing principles, policies and procedures.
