MAJOR PURPOSE: RN is responsible for overseeing comprehensive resident care plans through clinical assessment, review of resident's medical history, personal interviews, and completion of MDS. Responsible for coordinating interdisciplinary resident care conferences with Social Services staff.
PRIMARY FUNCTIONS: - Coordinates and completes the MDS with appropriate CAAs and trigger worksheets; determine assessment reference dates.
- Reviews the resident's medical history.
- Interview appropriate unit staff.
- Reviews resident care plans using the information available and resident care plan library during the MDS completion window.
- Daily review of rehab software for unscheduled assessments.
- Updates the resident care plan library, as necessary.
QUALIFICATIONS: - RN in the State of New York.
- Has a working knowledge of MDS and PPS process
- Knowledge of Submission Process and Interpretation of Validation ReportsMDS certification preferred.
