Job description
COMFORTING HOME CARE BY PHOEBE (CHCP) is seeking a full-time Nurse Supervisor (RN/LPN) for our Lehigh/Northampton region in our Bethlehem office. The primary role will be to coordinate home care services to allow clients throughout the region to maintain their independence in their own homes. Responsibilities will include supervising and training office staff and Caregivers, providing in home assessments and consultations with caseworkers, clients, and their families.
The Nurse Supervisor (RN/LPN) will serve as the primary liaison with Phoebe Ministries, PA DOH, OLTL, and other government agencies. In addition to being an excellent nurse, the ideal candidate will have outstanding people and management skills. This is a career track position offering the potential for promotion to Nurse Manager based on performance and continued growth of this location.

Nurse Supervisor Responsibilities:
- Serves as the lead nurse for CHCP's Bethlehem office providing leadership to office staff and caregivers.
- Manages the Client's Care Plans, Client's Schedules, Intake Assessments, Office Staff, Caregivers, and the hiring process for region.
- Manages all new home care inquiries from Phoebe and the community. This will include participating in meetings for discharge planning from the hospital or skilled nursing care to home care.
- Serves as the primary point of contact for Phoebe Allentown, PA DOH, and other government agencies.
- Provides nursing and home care education to clients, family, staff, caseworkers, and the public. This will include occasional presentations to groups, trainings, as well as participation at health events.
- Maintains a positive attitude and quality customer service with Clients, Co-Workers, and Phoebe staff.
- Interfaces with the Berks Area CHCP Nurse Manager.
- Manages office staff including, Assessment Nurses, Recruiter, and Care Supervisors who directly supervise direct care staff, scheduling, on call and other care responsibilities.
- Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.
- Responsibilities will also include interviewing, hiring, and training employees; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems and completing supervisory visits.
Requirements:
- PA licensed RN or LPN required.
- Minimum of 1 year work experience as a professional nurse; geriatric experience is preferred.
- Possession of a current driver's license and access to reliable transportation.
