Job description
Compass Health Network is searching for a Nurse Care Manager Service Coordinator to work at our location in Jefferson City on Christy Drive. They will serve as a conduit between CHCCM and Compass Health Network to make referrals into each organization to best meet the needs of the patient served. This individual will identify needed services, make appropriate referrals, provide warm hand-offs where possible, arrange appointments and follow-up on missed appointments as needed. This is a full time/benefits eligible position.
Other responsibilities of the NCM Service Coordinator:
Collaborates with CHCCM medical team to identify patients appropriate for psychiatric services to include Psychiatric Services, Psychiatric Rehabilitation, Therapy and/or educat
ional classes
Works with Compass Health Psychiatrists, and CHCCM Providers in developing and continually enhancing systems to support Population Based Psychiatry
Reviews client charts and consults with Psychiatry to identify clients who appear able to be transitioned to Primary Care for monitoring and medication management for behavioral health issues
Meets with patients to assess needs, determine levels of stability and assist in medical collaborative plan to transfer care as appropriate
Arranges psychiatric appointments and provides follow up to the psychiatry department regarding the Kept status of these appointments
Staffs patients with Medical team in anticipation of transitioning to Primary Care for psychiatric medication services
Arranges referrals for Psychiatric Rehabilitation and/or Therapy services as appropriate
Staffs with Compass Health Psychiatric Rehabilitation teams to coordinate care for psychiatric patients served at CHCCM
Staffs with Compass Health teams to identify clients needing Primary Care
Arranges Primary Care appointments and provides follow up to CHCCM regarding the Kept status of these appointments
Assists patients with identifying community resources and placing referrals or providing education on community resources
Coordinates with CHCCM and CHN staff regarding hospitalization and ED medication reconciliation and follow up appointments
Meets with new CHCCM staff to provide education about Compass Health programs
Collaborate with CHCCM and CHN staff to obtain lab work as needed
Assist Psychiatry with correspondence as needed
EDUCATION
Associate"s degree in nursing required
Bachelors degree in nursing preferred
WORK EXPERIENCE
At least two (2) years experience preferred
LICENSURE/CERTIFICATION
RN, licensed in Missouri required
