Requirements:
Active CA License
BC/BE
BLS PALS
Active DEA
PAs must have CA Controlled Substance Educ Course Cert
NPs must have Furnishing License w/ Schedules II
Immunizations such as Flu Vaccine TB Status/XRay Results
COVID vaccination proof (Primary series booster regardless of the date received OR 1 dose of current COVID vaccine (2023/2024 formulation)
At least 3 yrs of experience in similar setting
