The Social Worker BSW is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services and resources to ensure quality patient outcomes and appropriate utilization of health care and social services and resources. This role is responsible for supporting the navigation team to provide seamless care delivery and management of the complex social needs of the patient and their family. The Social Worker’s goal is to optimize patient outcomes, increase communication between facility, providers, patient, and family, decrease hospital readmission and ED visits, and retain patients within the Community Health Network service offerings for subsequent visits while collaborating with the patient’s primary care provider(s).

- LBSW in the State of Indiana required.
- Bachelor of Social Work in social services related field required.
- 1 year of current clinical experience or experience commensurate with the role preferred.
- Develop and nurture a collaborative partnership with the patient/caregiver, post-acute facilities and other Community Health Network divisions/departments.
- Create a seamless path for the patient/caregiver to answer questions or concerns, provide ongoing education, and navigate the patient through the care continuum.
- Empower patients to be active participants in their care and decisions regarding their care through education and action plans.
- Connect the patient to CHNw and community based resources.
- Conduct medication reconciliation at admission and discharge to SNF to assure safety and continuity.
- Bridge the communication gaps that can exist when patients transition from one care setting to another.
- Serves as a leader in providing excellent service to all customers following Community Health Network’s behavioral expectations, clinical standards, and patient satisfaction goals.
- Assumes accountability and responsibility for implementation of evidenced based protocols focused on increasing patient/caregiver ability to manage their care.
- Provides focused education on disease management ensuring optimum outcomes.
- Collaborates with interdisciplinary health care team including patients and families to anticipate needs and execute care management responsibilities and achieve optimal patient outcomes.
- Self-directed and able to organize and manage multiple tasks/projects simultaneously.
- Assumes responsibility and accountability for the management of resources across the episode of care while ensuring the plan of care is transitioned from acute care to SNF to the home setting to the physician office practice.
- Assures utilization of evidence based practice and appropriate standards of care and practice resultant in improved clinical outcomes and patient satisfaction.
- Holds self accountable for financial performance and clinical outcomes.
- Maintains responsibility for personal development and competencies necessary to improve organizational performance for assigned role.
- Incorporates theories and strategies to individualize care and drive behavioral change.
- Excellent analytical skills, including planning, organizing, communication and troubleshooting.
