We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented Full Time RN Admissions Nurse who will provide prompt and efficient access and admission of patients referred to Hospice of Central PA (HCP).
Job Summary:
Explains Hospice benefits in a manner consistent with the patient and family level of understanding. Responsible to communicate, collaborate and enhance relationships with referral sources with the overall goal of increasing admissions.
Qualifications:
- RN, with current PA license
- Current PA Driver's license
- Minimum of one year clinical nursing experience—hospice or home care experience preferred but not required
- Excellent communication skills [written, verbal, listening]
- Ability to track and complete multiple tasks at one time
- Ability to work independently; self-motivated
- Ability to work as part of a team
- Ability to work in a non-judgmental fashion with people of diverse backgrounds
- Actively supports HCP philosophy of care and Key Performance Standards of Excellence
- Proficient at basic computer/keyboard skills
- Knowledge and fluency of the Medicare Hospice benefits, Conditions of Participation, and industry as well as accreditation standards
Functions:
- In response to referrals, meets with patients and families to introduce the concept of hospice, explain the hospice benefit and facilitate admission to services
- Makes routine visits to facilities to assist in discharge planning, making symptom management recommendations and collaborate with facility staff for coordination of the plan for the patient
- Manages all aspects of care for patients under General Inpatient Benefit (GIP) while in contracted facilities
- Facilitates smooth transition of patients from inpatient to home or to the Hospice Residence
- Meets with facility professionals to acquaint them with hospice care philosophy and services provided by HCP
- Documents patient needs, interventions, recommendations in compliance with organizational policy and federal and state requirements as well as industry standards
