DESCRIPTION:
As an LVN Case Manager, you will be responsible for overseeing a caseload of 50-70 pediatric patients with moderate to high-acuity needs. Your primary role will involve clinical coordination of patient services, identifying comprehensive healthcare needs, and collaborating with a multidisciplinary healthcare team to ensure effective care delivery. You will also play a vital role in managing patient goals, risk stratification, utilization management, and optimizing cost-efficient care options. Additionally, you will assist with patient enrollment, maintain relationships with key healthcare organizations, and stay proficient in various technical platforms to facilitate efficient care coordination. This role may require home and community-based visits as needed to support patients and the team.
Schedule: Monday - Friday, 5 days per week, 8 Hour shifts
Essential Functions & Deliverables:
- Engage and manage a caseload of 50-70 moderate to high-acuity pediatric patients while also overseeing clinical coordination of patient services, linkage across organizations and services, medical collaboration, and care review.
- Identify patient needs across the healthcare spectrum, including a comprehensive review of medical, behavioral, and social needs, and collaborate with providers and healthcare team members across the healthcare continuum to ensure patient needs are comprehensively communicated across healthcare organizations and payers, and to negotiate care gaps and resolve issues arising during coordination of care.
- Collaborate with the RN to develop plans of care, stratify risk levels, and address patient needs and conditions.
- Identify the appropriate level of care, apply utilization management methodologies, monitor utilization and high-cost resources, provide feedback, and collaborate with the team to identify cost-efficient options.
- Collaborate with RNs to manage critical values, crisis situations, and escalate health concerns appropriately, while also evaluating and prioritizing care options and resources.
- Engage and enroll new patients into the ECM program while assisting with eligibility and patient recruitment, completing screenings, and monitoring benefits periods.
- Collaborate with team members to create and deliver systematic case reviews.
- Maintain working knowledge of insurance, county, and community-based resources to aid in continuity of care.
- Developing and maintaining relationships across organizations, while identifying beneficial resources and linking patients to care, is a crucial aspect of our work.
- Develop and continuously update department policies, procedures, and guidelines to ensure they remain aligned with program changes and contractual updates.
- Home and/or community-based visits may be necessary based on patient and team needs.
- Other duties as assigned by the Director of Quality Care.
QUALIFICATIONS:
- Education: Graduate of an accredited LVN program. You may be able to substitute LVN School Accreditation with Completion of CA State equivalency method by those who have acquired formal nursing education and met the hours and requirements listed by the CA Board of Vocational Nursing.
- Licensure/Certification: Current and active LVN license with the CA Board of Vocational Nursing.
- Experience: Minimum of 3 years of clinical experience, which should include at least 1 year of working directly with the pediatric population. Preferred qualifications include previous experience with Quality programs and prior Case Management experience. As a requirement of this position you must receive EPIC certification for the module you have been hired into.
- Essential Technical/Motor Skills: Must have strong technical skills and simultaneously utilize multiple platforms, including Electronic Medical Records, WebEx Contact Center, Care Management, and database systems.
- Interpersonal Skills: Work in a team to accomplish specific goals, new procedures, demonstrate use of equipment/supplies, communicate by phone/in person with department staff and outside sources. Display assertiveness and leadership capabilities, effectively balancing administrative duties, client needs, and task management. Be highly organized, detail-oriented, and proficient in presentation and communication skills.
- Essential Physical Requirements: SEE PHYSICAL DEMANDS.
- Essential Mental Abilities: Able to perform basic mathematical calculations and handle multiple duties simultaneously. Strong organizational and problem-solving skills. The ability to work well under pressure and maintain a calm demeanor.
- Essential Sensory Requirements: Recognize faces, hear on the phone/in person/in the environment and see or hear equipment indicators. Adequate visual, hearing, and tactile acuity to proficiently perform essential duties.
