Job description

Providing social work support services for children and young adults with complex medical needs and developmental delays in a progressive 83-bed pediatric long-term care setting. 

Assists in the planning, organizing, and implementing the overall operations of the Department of Social Work in accordance with current governmental regulations and standards and with Seven Hills Pediatric Center policies and procedures, and as may be directed by the Director of Social Work to ensure that medically related emotional and social needs of the residents and their families are met. Provides social work services for an assigned group of residents and their families.

Responsibilities include, but are not limited to, assuring the medical and emotional social needs of the residents and their families are met, coordinating and facilitating interdisciplinary team care planning meetings, maintaining appropriate social work documentation, staff training and special projects.

Assist in collection of diagnostic information and coordinate preadmission activities to ensure timely and smooth admissions.
Maintain regular contact with families and facilitate communication between families and staff to ensure maximum family participation in residents’ lives.
Refer families to other agencies and resources as necessary or requested.
Facilitate initial, quarterly and annual individualized service plan meetings.
Prepare and communicate with families/guardians individual goal summaries and planning.
Participate in medical and/or departmental rounds to reflect family/guardian perspectives and ensure that families/guardians remain informed.
Liaison to state agencies such as DPH, DCF and DDS when needed and appropriate.
Assist families and staff in effecting well-coordinated transfers to acute care settings or home.
Coordinate efforts to secure legal guardians for residents as they approach legal majority or whenever a guardian may be needed.
Assist families and staff in determining when residents’ needs would be more ideally addressed in an alternative setting, and prepare and implement discharge plans.
Counsel and support families, guardian and staff when a resident dies.
Identify financial resources for residents, their families and Seven Hills Pediatric Center.
Maintains appropriate social work documentation including admission, annual, quarterly and discharge summaries as well as periodic progress notes.
Assist with social work activities such as educational programs, social gatherings, and parent groups.
Participate in conferences and other continuing education opportunities to enhance skills and maintain licensure.
Participate in daily meetings with the Director of Social Work and social work staff.
Participate as the social work representative on facility committees.
Perform other duties as required or assigned.

 

Master’s Degree in Social Work, or a substituting combination of Bachelor’s level education and social work experience.
Must be licensed or license-eligible in Massachusetts.
Experience working with children and young adults with developmental delays and/or complex medical needs preferred.
Experience in a healthcare setting preferred.
Computer literate.
