For the Home Health Registered Nurses (RN) we offer the following compensation package:
- $80,000 to $92,500 Annual Salary (Depending on Experience)
- $100 Weekly Travel Stipend Reimbursement for full time status.
- Health Insurance - 60% Employer Paid for Non-Tobacco Users - Offered After 90 Day Introductory Period.
- Dental Insurance - Offered After 90 Day Introductory Period.
- 16 Days of PTO - Accrues on 1st Day of Employment - Available after 90 Day
- Introductory Period and once accrued accrued.
- Paid Holidays - 6 Holidays per year after the 90 Day Introductory Period.
- 401K Retirement Package with 4% Employer Match based on a vested schedule - available after 1 Year and 1,000 Hours worked in the same year.
- iPad Provided for Documentation.
- Supplemental Insurance available after the 90 Day Introductory Period – Accident
- Critical Illness, Hospital Confinement Indemnity, Short-Term Disability, Cancer/Specified Disease, and Whole or Term Life, etc.
Responsibilities:
- Provides home care nursing visits assigned.
- Submits completed documentation of the home care visit within the established time parameters.
- Attending multidisciplinary patient care conferences as requested.
- Instructs patients and caregivers according to the plan of care.
- Admits patients and develops the plan of care adhering to the Medicare criteria for home care provisions.
- Makes nursing assessments and reports significant changes in the patient's condition to the appropriate team members.
- Communicates to physicians any changes in patient's condition.
- Obtains physician orders for changing patient care needs.
- Revises the plan of care based upon assessment findings.
Requirements:
- Required - Registered Nurse Licensed in the State of Pennsylvania
- Preferred - 1 + Years of Experience as a Home Health Nurse Preferred
- Required - 1 Year of clinical experience within a hospital, rehab or long-term care setting.
- Preferred - OASIS Experience - Training Offered
- Required - Active and Valid Driver’s License and Car Insurance
- Required - Excellent verbal and written communications skills.
- Covering Philadelphia
- Providing care to 5 - 7 Patients Per Day - 32 Points Per Week
- On Call 1 Weekend Per Month and 2 Holidays Per Year (Minimal for Emergencies).
